Status:                   Term Part-time (Maternity Leave Coverage)

Start Date:            October 20, 2025

End Date:             End date will align with the duration of maternity leave (anticipated 12–18 months from October 2025)

Pay Grid:               BCNU Nurse Grid - (LPN Wage Schedule) ($32.84 to $37.81 per hour + $2.15 hourly premium)

Summary of Duties:

Under the direction of the Nurse Manager, the Licensed Practical Nurse (LPN) - Chronic Disease provides health education and support to community members in preventing, managing, and monitoring of chronic diseases. The LPN - Chronic Disease advocates and facilitates access to health care providers, empowers community members to manage their own health and provides education and guidance related to advanced care planning.

 

Key Deliverables and Expectations:

  • Establish a therapeutic relationship with the client or family using interpersonal and interviewing techniques, in person and/or virtually, to ensure their choice and autonomy in decision-making and care planning.
  • Conducts comprehensive nursing assessments of clients and their families who would like to participate in the program.
  • Use their professional knowledge to work in partnership with the client and family to establish wellness goals. 
  • Use a holistic approach and interprofessional team model when using referrals, and links to other services to support client in optimal wellness outcomes.
  • Promotes client and family empowerment and independence using a strengths-based approach to health education, health monitoring and basic medication reviews.
  • Assist the client and their family in accessing medical equipment and supplies.
  • Health Education for general community members based on SMART Programming. 
  • Deliver direct care per BCCNP standards and Lil’wat Health and Healing guidelines.
  • Provide care management to support clients in managing their own care and navigating various health services, including assistance in accessing internal and external health supports and support groups.
  • Organize visiting health screening clinics such as mammography, mobile diabetes, pharmacists.
  • Provide Advance Care Planning education and support to the general community.
  • Assisting Community Health nursing team in communicable disease emergency related activities
  • Evaluates own practice and uses professional consultation, training, and professional resources to maintain strong practice standards and the provision of quality care to community members.
  • Perform other related duties as requested.

Experience and Attributes: 

  • Practical Nursing Diploma from a recognized university. 
  • Current practicing registration, in good standing, with the BC College of Nurses & Midwives.
  • Current CPR/AED certification for healthcare providers.
  • Minimum one year of experience in public health or community health settings.
  • Minimum one year of experience in adult or elder health.
  • Previous experience providing education around chronic diseases, advanced care planning and palliative care.
  • Demonstrated ability to work both independently and collaboratively as part of a team.
  • Excellent written and verbal communication skills in English.
  • Good skills in problem-solving, conflict resolution and de-escalation.
  • Good interpersonal skills with the ability to build relationships with children, families, staff and external partners.
  • Knowledge of First Nations issues, families, family systems and culturally safe practices. 
  • Understanding of the impacts of colonialism, including residential schools, trauma, family breakdown and poverty. 
  • Strong organizational skills, meticulous attention to detail and time management skills.
  • Highly professional in all aspects of business, presentation and interaction with all stakeholders, clients, and Members, while maintaining confidentiality at all times.
  • Experience working in a First Nations community and willingness to learn and incorporate Lil’wat Nation’s culture, traditions and protocols. Experience working in a rural setting an asset.
  • Valid BC Class 5 Driver’s License, a clean Driver’s Abstract, and a reliable vehicle.
  • Must be able to provide a clear Criminal Record Check, including the vulnerable sector.
