Pay Grid: Professional ($38,038 - $53,599 per year)
Reporting to: Maternal Child Health Manager
Summary of Duties:
As part of the Maternal and Child Health Team the Family Mentor provides outreach and support to pre and post-natal clients, children 0-6 years of age, and their families.
Key Deliverables and Expectations:
- Provides one-to-one service with pre- and post-natal clients including education, support, and home visits.
- Refers and assists families to access internal and external resources and services.
- Provides transportation to appointments, shopping, banking, food bank, and others, as required.
- Advocates for children and families.
- Assists families to ensure their infants and children are registered with Vital Statistics, Child Tax Benefit, Medical Services Plan, and Social Insurance Number.
- Assists with the distribution of supplies such as car seats, breast pumps, and food vouchers.
- Plan and deliver Baby Clinics, Prenatal Classes, parenting groups, and Maternal and Child Health (MCH) program recreational and cultural activities.
- Support meal planning, purchasing, and cooking nutritious and economical meals for group sessions.
- Collaborates with other Lil'wat programming to offer community education and attend events such as health fairs, celebrations, and workshops.
- Innovate and assist in developing MCH initiatives and programs.
- Perform other related duties as requested.
Experience and Attributes:
- Maternal and Child Health home visitor training.
- Baby Safe/ CPR/AED first aid.
- Breastfeeding support training.
- Valid Food Safe certificate.
- Experience with parenting programs such as Nobody's Perfect Parenting.
- Growing Great Kids, Talk Learn & Grow, etc.
- Experience with Microsoft Office; Word, Excel, etc.
- Strong written and verbal communication.
- Clear Criminal Record Check, including the vulnerable sector.
- Valid driver’s license and access to a vehicle.
- A good understanding of Lil’wat Culture and Traditions.
- A willingness to seek the above training and/or an equivalent combination of education, training, and experience will be considered.
